ASKED: AI against premature ejaculation

The first artificial intelligence in the field of sexual counseling in German-speaking countries is launched – and MedLabPortal wanted to know exactly what it means. The background: Beatrix Roidinger and Best Lover – Institute for Male Sexuality will launch “Best Lover CONTROL”, an AI-supported 30-day intensive program against premature ejaculation, on May 26, 2026.

Ms. Roidinger, premature ejaculation affects up to 30 percent of all men. Nevertheless, hardly anyone talks about it. Why is that?

Roidinger: Because the topic is filled with enormous shame. Many men define themselves sexually through control and perseverance. Those who come too early perceive it as a personal failure. In addition, there are hardly any rooms in which men talk openly about sexual problems. Not with friends, often not even with your partner. So they sometimes remain silent for years. And the pressure of suffering is enormous.

And these men are now supposed to talk to an AI about their sex lives?

Roidinger: Exactly. That sounds paradoxical at first, but it works. The inhibition threshold to write to an AI is much lower than that of opening up to a stranger. There is no counterpart who could judge. You can write at three o’clock in the morning if you can’t sleep. You don’t have to make an appointment, enter a waiting room, look no one in the eye. For many men, this is the first step in dealing with the topic at all.

What exactly is Best Lover CONTROL?

Roidinger: A 30-day intensive program that consists of two components. First, an extensive video course in which I teach all the content. Second, a digital sex counselor, whom we call AI Bea, my digital alter ego, so to speak. The AI Bea guides the participant through the entire process. It can be reached via Telegram, i.e. like a normal chat on a smartphone.

What makes this AI different from ChatGPT?

Roidinger: That’s the most common question we get. And the answer is: No. With ChatGPT, Gemini or other providers, the user asks the questions. This presupposes that you know what to ask. That’s exactly the problem. Most of those affected do not know what they do not know. They google “hold out longer”, try some breathing techniques or pelvic floor exercises and fail. Not because the techniques are wrong, but because individual exercises do not work without a guided, structured process.

Our digital sex counsellor works differently. She is leading the trial. She actively asks questions, recognizes where the client stands, sends the right exercise at the right time, provides background knowledge and explains exactly how the exercise works. That’s the difference between a classic ChatBot and our AI.

So is it not only about the exercises, but also about the process of writing itself?

Roidinger: Absolutely. This is often underestimated. Just the fact that someone regularly asks, “How was the exercise?” “What did you feel?” “What was difficult?” does something to the participants psychologically. You are encouraged to self-reflection, you have to put into words what you have experienced. Writing it down is part of the process. In counselling, we know that if you say or formulate something, you process it differently than if it is only (often unconsciously) circling in your head. This is exactly what happens in the chat with the AI Bea. You are in contact, you are accompanied, you stay tuned. This is not a technical feature, but a therapeutic principle that we have deliberately built into Best Lover CONTROL.

How was Best Lover CONTROL trained?

Roidinger: We have put all the expertise of our team into AI. The complete course structure, our exercise catalog, the consulting logic, i.e. when which step comes and why. The AI knows our methodology: pelvic floor training, sexologically guided masturbation, nervous system regulation, changing arousal patterns. She does not work keyword-based, but understands the therapeutic process behind it.

That sounds like a long development process. How do you make sure that your AI really reacts the way a human advisor would?

Roidinger: That was actually the most complex part of the development. Feeding an AI with expertise is one thing. But getting them to adhere to our consulting logic exactly was difficult. The biggest challenge is that we don’t know beforehand what the participant is writing. One responds with one word, the other sends three paragraphs. One is enthusiastic after an exercise, another is frustrated because nothing happened. Yet another asks a question that is completely off topic. AI has to react to all of this correctly, just as we would do as human advisors. So don’t stubbornly send the next exercise, but first ask, classify, motivate or even brake. We tested, adjusted, discarded and reset. Always with the question: Would I, as a sex counselor, react the same way at this point? Would I give this exercise now or is the participant not ready yet? The result is an AI that advises the client with a structured process and provides feedback and explanations where necessary.

Can you briefly explain what is happening in terms of content? What do the participants learn?

Roidinger: In essence, it is about establishing a new arousal pattern. Most men with premature ejaculation have trained themselves over years to develop a pattern in which arousal increases very quickly and is almost exclusively concentrated in the genital area. The tension is very high, both physically and mentally. This then leads to the fact that he quickly loses control over his arousal and thus also over his ejaculation. So we teach men to perceive their arousal in a differentiated way and to distribute it throughout the body. With Best Lover CONTROL, you will learn step by step to get control over your premature ejaculation.

This is practiced, among other things, by changing masturbation and by working with erogenous zones all over the body.

That sounds like a process that actually needs personal support. Can AI really do that?

Roidinger: That’s why it’s a hybrid model. The content comes from real people, from my team and me. The AI takes care of the accompaniment, the daily structure, the reminders and the motivation. In our individual consultations, this process takes an average of 10 to 12 sessions over several months. With the AI, the participants can do it in 30 days, because the support is not limited to one appointment per week, but takes place permanently.

Suppose a man regains control. Has the goal been achieved?

Roidinger: Not quite. Of course, men come to us because they suffer massively from the problem of “premature ejaculation” and want to get rid of this burden. This is our core competence and this is what you can expect with our program. But what many people don’t expect is that when we work on the arousal pattern, much more happens than just “being able to last longer”. Men learn to perceive their bodies in a completely different way during sex. This changes not only the duration, but the overall quality. Many of our clients report according to Best Lover CONTROL that their orgasms become more intense, that they can really enjoy sex for the first time. This is a double reward, so to speak: the pressure of suffering goes away, and what comes instead is a sex life that feels fundamentally better than before.

Will AI replace human sex counseling in the long term?

Roidinger: No, and that’s not the goal either. There will always be topics that need human accompaniment. Such as profound couple conflicts, traumatic experiences, complex psychological relationships. But for a clearly defined process such as working on the excitation pattern, AI is an efficient lever. It makes professional help accessible to men who would otherwise never have dared to take the step into counselling or for whom classic sex therapy that lasts for months would simply be too expensive and too time-consuming for many.
