New S3 guideline recommends six months of full breastfeeding and at least twelve months of total breastfeeding
A new S3 guideline entitled “Breastfeeding duration and interventions to promote breastfeeding” has been published. For the first time in Germany, it contains evidence- and consensus-based recommendations on the duration of breastfeeding.
The guideline was developed under the leadership of the German Society for Pediatrics and Adolescent Medicine (DGKJ), the German Society for Gynaecology and Obstetrics (DGGG) and the German Society for Midwifery Science (DGHWi). A total of 26 scientific societies, professional groups and representatives of breastfeeding mothers were involved. It was created as part of the National Strategy for the Promotion of Breastfeeding.
For full-term infants, it is recommended to breastfeed exclusively or predominantly (full breastfeeding) for the first six months of life. Exclusive breastfeeding means nutrition exclusively through breast milk, predominant breastfeeding also only allows water or tea. This recommendation received a B grade.

In addition, a total breastfeeding period of at least twelve months is recommended. This recommendation was adopted with grade A. Partial breastfeeding supplemented with infant formula or complementary food is taken into account.
The recommendations are based on a systematic literature review and evidence assessment according to the GRADE standard. 30 health-relevant endpoints in mother and child were analyzed, including infectious and metabolic diseases in the child as well as aspects of maternal health.
The guideline creates a uniform basis for advice for health professionals and resolves previous deviations from international recommendations such as those of the WHO. She emphasizes that the individual situation of mother and child remains decisive.
The published part deals exclusively with the duration of breastfeeding. Recommendations on interventions to promote breastfeeding are to follow in a further step.
