Record number of drug risks: Over 11,000 suspicious activity reports from pharmacies
The Drug Commission of German Pharmacists (AMK) has registered a new high in reports of suspected drug risks in 2025. A total of 11,154 reports were received – more than in previous years (2024: 10,882, 2023: 8,320). The reports came from 4,978 different (hospital) pharmacies.
About two-thirds of the reports concerned quality problems, and one-third adverse drug reactions. Reports of medication errors have increased significantly: 624 cases in 2025 compared to 348 in the previous year. In contrast, there was a slight decline in reports of suspected drug abuse (2025: 41, 2024: 50).

“Local pharmacies ensure more safety in drug therapy and thus increase the protection of patients,” explained Prof. Dr. Martin Schulz, Chairman of the AMK. The Commission evaluates and processes the reports of the pharmacy teams and has been informing pharmacies about relevant risks for more than 50 years. In 2025, AMK issued a total of 263 risk information, including 23 Red Hand and Information Letters and over 150 batch-related recalls and reviews.
Dr. Armin Hoffmann, President of the Federal Chamber of Pharmacists and member of the AMK, emphasised: The figures impressively show the central role of pharmacists in assessing quality issues, identifying possible application errors and providing advice on the tolerability of drug therapies. “Without pharmacies, the supply of medicines is not possible.”
The increasing number of reports is seen by the AMK as an expression of a high level of sensitivity and attention on the part of local pharmacies. At the same time, it underlines the importance of pharmaceutical risk management in Germany.
