WHO: Mental health must become a central component of NTD care

For the first time, the World Health Organization (WHO) has published a comprehensive, evidence-based care package that firmly integrates mental health and stigma reduction into programs against neglected tropical diseases (NTDs). The “Essential Care Package to address mental health and stigma for persons with neglected tropical diseases” is intended to support governments, health leaders and frontline services in systematically addressing psychological stress and stigma.

NTDs affect over a billion people worldwide, especially in poor regions. Those affected are more likely to suffer from depression, anxiety disorders, psychological stress and suicidal behaviour – caused not only by the disease itself, but also by stigma, discrimination and social exclusion. Without adequate psychosocial support, treatment remains incomplete and the elimination of the diseases becomes more difficult.

The new package defines clear recommendations for action on several levels:

  • For those affected: Recognizing psychological stress, knowledge about support services, peer support and knowledge of the rights to health care, work and social participation.
  • For families and communities: Early recognition of stress, support in seeking help and reducing stigmatizing attitudes.
  • For health professionals: Routine, compassionate, person-centered care with screening, psychoeducation, and clear referral pathways to peer support, primary care, and specialty psychiatry. Training courses are intended to reduce stigmatising attitudes among staff.
  • At the system level: Coordinated planning between NTD and mental health programs, integration of mental health into NTD data collection, and establishment of collaborative care models.

The package emphasises that integration only works if it is seen as a core component – and not as an add-on. It is designed for low-resource settings and aims to improve adherence, reduce isolation, and advance the elimination of NTDs.

The Essential Care Package was developed by the WHO together with a broad international alliance, including Liverpool School of Tropical Medicine, NTD NGO Network, CBM Global, The Leprosy Mission, leprosy and affected organizations. It is the first official WHO document to define mental health and stigma as an integral part of NTD care.
